Compile to Flat XML (.fodt) does compile headings only up to level 2
When you compile a test document to .fodt only the level 1 and 2 headings (# and ##) get properly compiled to level 1 and 2 OpenDocument headings. Onwards the headings are just text body enclosed by ### ### and so forth.
As this doesn't happen with other output formats I guess it is not the fault of my compile format.

Hm, can't confirm. I tried with my attached (zip compressed) test project, and I get Heading 3 with mmd>fodt.

Same here, with a simple 5-level test I get Heading 1 -- 5 styles assigned correctly in the .fodt file. If you're getting bare hashmarks, I'd take a look at a compiled MMD file itself and see if there is anything in settings that might be creating a confused result.
With the footnote that lacks data, if you could provide a sample that produces that result that would be great. That sounds like a potential Scrivener bug, but I don't know how to reproduce it.
All things relating to how stylesheets are assigned are matters of how MMD works. If there is a problem there, assuming it isn't an issue with how MMD is being generated from the compiler, then it's probably an MMD issue itself. That could be confirmed by processing the file directly on the command-line. I got "Default Style" assigned to all regular paragraphs.
